Do you need to see a signed liability form for a player? Below are steps to take to quickly access the liability waivers that your players' parents have signed:

  1. Log in to your Team Sports Admin Account. 
  2. Click on TEAMS in the main menu.
  3. Locate and click on the team of the player whose waivers you are looking for.
  4. Under Roster, find and click on the player.
  5. Scroll down to the Signed Documents section of the player profile to access their waivers.
  6. Under Actions, click on Download to get a PDF of the waiver.
    • If the form was submitted by the parent or player in person, it will say "Offline" under "Actions". You can click on "Offline" to find out when the form was submitted and read any notes entered about it.
    • If no forms have been submitted at all, the "Signed Documents" section will say "No Documents Found" in red.
